Requesting Your Health Care Records
To request a copy of your health care records from The Hour Therapy, PLLC:

1. Send an email to danielle@thehourtherapy.com or call 713-487-7259.

2. Our team will provide you with an “Authorization to Request or Disclose Client Information” (Release of Information/ROI) form to complete.

3. Complete and return the ROI form.

4. The practice will review the record and make any required redactions, if applicable.

Tip: To avoid delays, make sure your ROI form is fully completed and signed.
